Two hundred forty-eight deputies voted for his appointment, 201 voted against, and none abstained.
Previously, the government of Prime Minister Mateusz Morawiecki did not receive a vote of confidence.
The new prime minister said he is in debt to those who trusted the "new hope" during the October 15 elections.
Earlier, Tusk promised to unblock the border with Ukraine and end the disputes between Warsaw and Kyiv.
On December 14-15, he will participate in the EU summit in Brussels, where they will consider the possibility of starting negotiations on Ukraine's membership and allocating 50 billion euros of aid to it.
What is known about Donald Tusk
Tusk previously served as Prime Minister of Poland from 2007 to 2014 and later served as President of the European Council.
Tusk has already promised to take several measures to eliminate the consequences of Morawiecki's eight-year rule of the Law and Justice (PiS) party.
The new government has promised to restore the independence of the judiciary, which it says has been systematically undermined under previous administrations.
He also promised to unblock €36 billion in EU funds meant for Poland, which Brussels has refused to provide over concerns about the rule of law.
A court decision from 2020, which banned abortions in almost all cases, will also be overturned.
